How to Schedule a Service: Call 225-343-4232, but please note...
 
Speech & Language Evaluations:
To schedule a speech and language evaluation, please call The Foundation office at 225-343-4232 to make an appointment. Please indicate whether the person to be tested is also hearing-impaired. A speech/language evaluation takes about an hour and a half. If the evaluation is for a preschool-aged child, we will schedule a hearing screening as well. Together the speech and hearing evaluations will take about two hours. You will need to arrive about fifteen minutes early for any appointment.

Individual Language Therapy (Adults & Children), Group Language Therapy & Group Language Therapy with DTT/ABA(for preschool children):
Before being placed in individual or group language therapy (with or without Discreet Trial Training), an evaluation performed within the last three months by a licensed speech therapist must be sent to the Supervisor of Clinical Services at BRSHF. Please mail to: BRSHF, Attention: Clinical Supervisor, 535 West Roosevelt Street, Baton Rouge, Louisiana 70802, or hand deliver a copy to that address. If a current evaluation is not available, one should be scheduled at The Foundation. (See above.) The Clinical Supervisor will review the evaluation, then call you to discuss next steps in scheduling appropriate services, if they are needed.

Audiology Services:
Hearing Tests can be scheduled for adults at either the Foundation’s Roosevelt Street campus or the Denham Springs office. Hearing tests for children under four years of age must be performed at the Roosevelt Street location. Hearing tests are by appointment only. Call The Foundation office at 225-343-4232 to schedule. The test will take about an hour. You will need to arrive about fifteen minutes early.

If you want to schedule an appointment to get hearing aids, you must send the Audiology Department a comprehensive hearing evaluation completed by a licensed audiologist within the last six months. Please mail to: BRSHF, Attention: Audiologist, 535 West Roosevelt Street, Baton Rouge, Louisiana 70802, or hand deliver a copy to that address. If you do not have results of a current hearing test, you may schedule one at The Foundation or elsewhere. (See above.) The Foundation must have the evaluation in hand before a hearing aid consultation is scheduled.

If you are Medicaid eligible, your physician must FAX a Community Care Form to the Foundation at 225-343-4233, before an appointment can be scheduled for any service. When you come to your first appointment, don’t forget to bring your current Medicaid card with you.
